Corns vs. Calluses — What’s the Difference?

Calluses are broad, diffuse areas of thickened skin that form in response to repeated pressure or friction. They most often appear on the ball of the foot, the heel, or the outer edge of the big toe. Calluses are the body’s protective response — but when they grow too thick, they become painful.

Corns are smaller, more focused areas of hardened skin with a dense central core that presses into deeper tissue. Hard corns typically form on the tops or sides of toes. Soft corns — white and rubbery — form between toes where moisture is trapped.

Important: Never use medicated corn pads or razor blades to remove corns yourself — especially if you have diabetes or poor circulation. These methods cause skin damage and increase infection risk significantly.

However, for patients with diabetes, peripheral neuropathy, or poor circulation, even a minor corn or callus can develop into a serious ulcer or infection. No. Using razor blades, scissors, or medicated corn pads to self-treat is not safe — particularly for patients with diabetes or compromised circulation. Medicated pads contain salicylic acid, which can break down healthy surrounding skin and cause open sores. Corns and calluses are areas of thickened skin from pressure and friction; they have no root, no distinct border, and no skin line interruption. Plantar warts (verrucae) are caused by human papillomavirus (HPV), often appear with tiny black dots (capillary thromboses), disrupt normal skin lines, and are painful when pinched from the sides rather than pressed directly. Treatment is completely different — a podiatrist can distinguish them on exam.

Recurrence happens when the underlying cause — abnormal pressure or friction from gait mechanics, shoe fit, or structural deformity — is not addressed. Debridement alone provides temporary relief. Long-term prevention requires correcting the mechanical cause through custom orthotics, footwear changes, or in some cases, addressing an underlying deformity like a bunion or hammertoe.

Routine debridement for corns and calluses is generally covered by Medicare and most private insurance plans when there is a qualifying systemic condition (diabetes, peripheral vascular disease, peripheral neuropathy).
