Types of Foot & Ankle Arthritis

Osteoarthritis (OA) is the most common form — caused by the gradual breakdown of cartilage in the joints of the foot over time. The big toe joint, midfoot (tarsometatarsal joints), and ankle are most frequently affected. Pain and stiffness worsen with activity.

Rheumatoid arthritis (RA) is an autoimmune condition that causes joint inflammation and can affect multiple joints in the foot simultaneously, often symmetrically. RA can cause deformities like bunions, hammertoes, and flat feet over time.

Post-traumatic arthritis develops in joints that were previously fractured or dislocated, even years after the original injury.

The earlier arthritis is diagnosed and treated, the better the long-term outcome. Conservative treatment is highly effective for most patients — surgery is rarely the first step.

Arthritis Treatment Options

First-Line

Custom Orthotics

Custom foot orthotics control abnormal motion, redistribute joint stress, and significantly reduce pain in arthritis patients — often the most effective non-surgical intervention available.

Injections

Corticosteroid Injections

Targeted cortisone injections into arthritic joints provide powerful anti-inflammatory relief, typically lasting weeks to months. PRP (platelet-rich plasma) injections are an option for patients seeking regenerative approaches.

Non-Surgical

Bracing & Footwear Modifications

Specialized braces, rocker-sole shoes, and carbon fiber shoe inserts unload arthritic joints, reduce pain with walking, and may slow the progression of joint damage.

Surgical Option

Minimally Invasive Surgery

When conservative care is no longer sufficient, our podiatric surgeons offer joint-sparing procedures, arthroscopic debridement, and — when necessary — joint fusion (arthrodesis) to eliminate pain and restore function.

Frequently Asked Questions

Early foot arthritis commonly presents as morning stiffness that improves with movement, aching pain after prolonged activity, swelling or warmth in one or more joints, and subtle changes in the way you walk (a limp or altered gait to avoid painful positions). The big toe joint and midfoot are most frequently affected. A podiatrist can confirm arthritis with weight-bearing X-rays and a clinical exam.

Yes — the vast majority of foot arthritis patients are successfully managed without surgery. Conservative treatment including custom orthotics, anti-inflammatory medications, physical therapy, activity modification, and targeted corticosteroid injections provides significant pain relief for most patients. Surgery is typically reserved for cases where conservative care has failed after 6 to 12 months.

For most types of foot arthritis, shoes with a wide toe box (to avoid compression), a stiff or rocker-bottom sole (to reduce motion through painful joints), and good cushioning are recommended. Avoid high heels, narrow toe boxes, and thin flat soles. Your podiatrist can recommend specific footwear characteristics or shoe brands appropriate for your arthritis type and location.

Moderate walking is generally beneficial — it maintains range of motion, reduces stiffness, and supports joint health. However, high-impact activities (running, jumping) can aggravate inflamed joints. The key is finding an activity level that keeps you moving without flaring symptoms. Custom orthotics and appropriate footwear significantly expand what most arthritis patients can do comfortably.

Corticosteroid (cortisone) injections deliver powerful anti-inflammatory medication directly into an arthritic joint, providing pain relief that typically lasts weeks to months. They are most effective for joints with active inflammation (swelling, warmth, tenderness). The number of injections per year is limited to avoid potential cartilage effects; your podiatrist will discuss whether injections are appropriate for your specific situation.

Don’t Let Arthritis Slow You Down.

Effective arthritis treatment starts with an accurate diagnosis. Our Chicago podiatrists will evaluate your joints, review your imaging, and design a treatment plan that targets your specific arthritis type and severity.
