An ingrown toenail (onychocryptosis) occurs when the edge or corner of a nail curves downward and grows into the surrounding skin. Understanding the root cause helps prevent recurrence after treatment.
Improper nail trimming — cutting nails too short or rounding the corners — encourages the nail edge to dig into the skin as it regrows. Tight or ill-fitting footwear with a narrow toe box compresses the toes and forces the nail against the skin.
Other common causes include nail shape genetics (naturally curved or fan-shaped nail plates), toe trauma from stubbing or sports, toenail fungus that thickens and distorts the nail, and certain medications such as oral retinoids or chemotherapy agents.
Symptoms progress through stages. Early-stage ingrown toenails respond well to conservative care, but signs of infection require prompt professional treatment.
Tenderness or pain along one or both sides of the toenail, especially when pressure is applied by shoes, socks, or touch.
Redness and inflammation of the skin surrounding the nail, with swelling of the toe, especially at the tip.
Accumulation of fluid around the affected area, sometimes progressing to pus or discharge draining from the nail border.
Skin that feels warm and appears bright red, overgrowth of tissue (granulation tissue) at the nail edge, or foul odor emanating from the toe. Spreading redness or red streaks up the foot require emergency care.
