Ingrown toenail treatment Chicago — professional podiatric nail care

What Causes Ingrown Toenails?

An ingrown toenail (onychocryptosis) occurs when the edge or corner of a nail curves downward and grows into the surrounding skin. Understanding the root cause helps prevent recurrence after treatment.

Improper nail trimming — cutting nails too short or rounding the corners — encourages the nail edge to dig into the skin as it regrows. Tight or ill-fitting footwear with a narrow toe box compresses the toes and forces the nail against the skin.

Other common causes include nail shape genetics (naturally curved or fan-shaped nail plates), toe trauma from stubbing or sports, toenail fungus that thickens and distorts the nail, and certain medications such as oral retinoids or chemotherapy agents.

Important: If you have diabetes, peripheral vascular disease, or a condition affecting your immune system, even a mild ingrown toenail requires same-day professional care. Reduced sensation can mask the severity of infection.

Symptoms of an Ingrown Toenail

Symptoms progress through stages. Early-stage ingrown toenails respond well to conservative care, but signs of infection require prompt professional treatment.

Tenderness Along the Nail

Tenderness or pain along one or both sides of the toenail, especially when pressure is applied by shoes, socks, or touch.

Redness & Swelling

Redness and inflammation of the skin surrounding the nail, with swelling of the toe, especially at the tip.

Fluid Accumulation

Accumulation of fluid around the affected area, sometimes progressing to pus or discharge draining from the nail border.

Signs of Infection

Skin that feels warm and appears bright red, overgrowth of tissue (granulation tissue) at the nail edge, or foul odor emanating from the toe. Spreading redness or red streaks up the foot require emergency care.

Ingrown Toenail Treatment Options

Our podiatrists tailor treatment to the severity of your condition, your history of recurrence, and your activity level.

Mild / Stage 1

At-Home Conservative Care

Appropriate only for very early, non-infected cases. Should be combined with professional evaluation. Includes warm water soaks 3–4 times daily for 15–20 minutes, gently lifting the nail edge with clean cotton, wearing open-toed or wide-toe-box footwear, and over-the-counter topical antibiotics for minor irritation.

Moderate / Stage 2

In-Office Partial Nail Avulsion

A minimally invasive in-office procedure for most ingrown toenails. Performed under local anesthesia with no stitches and immediate weight-bearing. Local anesthetic injection to fully numb the toe, removal of the offending nail border (10–30% of nail width), thorough wound irrigation and antiseptic dressing. Walk out of the office immediately post-procedure.

Recurrent / Permanent Fix

Permanent Nail Correction (Matrixectomy)

Minimally invasive nail avulsion combined with chemical matrixectomy — the definitive solution for chronic ingrown toenails. Phenol is applied to the nail matrix to prevent regrowth. Over 95% long-term success rate with a cosmetically acceptable result. Walk out of the office immediately. Covered by most major insurance plans.

What is a matrixectomy? The nail matrix is the tissue at the base of the nail responsible for nail growth. A chemical matrixectomy uses phenol (or occasionally sodium hydroxide) to permanently destroy a narrow strip of matrix cells on the side of the nail that was causing the problem. This prevents that portion of the nail from ever regrowing, providing a permanent cure without the need for a second procedure.

Don’t Try to Remove an Ingrown Toenail at Home

YouTube tutorials and online forums are full of DIY ingrown toenail “fixes.” These methods are not only ineffective — they are genuinely dangerous and frequently turn a manageable problem into a serious infection that may require hospitalization.

  • Serious Infection Risk: Breaking the skin barrier with non-sterile instruments introduces bacteria directly into the wound. Cellulitis, abscesses, and in severe cases, osteomyelitis (bone infection) can result.
  • Worsening the Problem: Cutting a V-notch, tearing the nail, or trimming too aggressively typically makes the nail grow back in a more problematic shape.
  • Uncontrolled Bleeding: Attempting to remove a nail at home without anesthesia or proper surgical tools can cause significant pain and uncontrolled bleeding.
  • Guaranteed Recurrence: Home remedies never address the nail matrix. Without professional treatment, the ingrown toenail will almost certainly return, often more severely.

Cost & Insurance

Ingrown Toenail Removal Cost & Insurance

Ingrown toenail removal is a covered medical procedure under virtually all major insurance plans. It is billed as a surgical service — not cosmetic — so most patients pay only their specialist copay or a small coinsurance. Same-day treatment is available.

What Insurance Typically Covers

  • Partial nail avulsion (standard removal) — covered by most plans
  • Permanent correction (matrixectomy) — covered when recurrent ingrown
  • Medicare — covers ingrown nail procedures as medically necessary
  • Treatment of infected ingrown toenail — covered as a medical procedure

What Affects Your Out-of-Pocket Cost

  • ·Your plan’s specialist copay (typically $20–$60)
  • ·Partial vs. permanent correction technique
  • ·Whether anesthesia/anesthetic injection is billed separately
  • ·Culture or pathology if infection is present

Permanent correction (matrixectomy) may require documentation of recurrent ingrown nails. We capture this in your chart at each visit.

Frequently Asked Questions

Common questions about ingrown toenail removal at Michigan Avenue Podiatry.

The procedure itself typically takes 15 to 30 minutes from start to finish, including the time needed to administer local anesthesia and allow it to take full effect. Most patients are in and out of the office within an hour, including check-in and post-procedure wound care instructions.

The procedure is performed under local anesthesia, so you will feel only pressure — not pain — during nail removal. The injection of anesthetic may cause a brief, sharp sting lasting a few seconds. After the anesthesia wears off (typically 2 to 4 hours later), mild soreness for 24 to 48 hours is normal and easily managed with over-the-counter pain relievers such as ibuprofen.

A simple nail avulsion without matrixectomy carries a recurrence rate of approximately 70% over time. When a chemical matrixectomy (typically using phenol) is performed at the same time to permanently destroy the nail matrix cells responsible for that nail border, the long-term success rate rises above 95%. Our podiatrists will assess your individual history of ingrown toenails and recommend the most appropriate approach to minimize the chance of recurrence.

Yes. The vast majority of patients walk out of the office under their own power immediately after the procedure. We recommend wearing open-toed shoes or comfortable sandals to your appointment, and avoiding high-impact exercise or prolonged standing for 24 to 48 hours. You can return to normal daily activities the same day. Running and vigorous exercise can typically be resumed within 3 to 5 days, once soreness resolves.

In-office ingrown toenail removal is a covered benefit under most major commercial insurance plans, Medicare Part B, and Medicaid when the procedure is deemed medically necessary. Coverage is typically straightforward for infected or acutely painful cases. Our front desk team will verify your specific benefits and explain any expected out-of-pocket costs, such as copays or deductibles, before your appointment. Call (312) 701-0770 and we will be happy to run a benefits check for you.
